I'm afraid you have a very special problem there.
The mouse and keyboard are controlled by the IKBD, which is a separate mini-computer based on a Motorola 6301 with its own RAM and ROM.
This can be switched into several modes (IIRC) and when pressing mouse-buttons during power-up, it is switched into a mode that is undefined.
So to me it looks like your adapter is sending a mouse-key during power-up which initialises the IKBD wrongly.
If you can access the original mouse-connector when opening your 19" rack, try booting the computer without any mouse/joystick connected and see what happens.
