As far as I know APP is for passing peramiters to a TOS application (with a TOS extension these are generly text only).
Think of it like a DOS app, if you want to pass info to the prog then you add something like;
format a: /u
remember that GEM does not have a command promt to pass the info, running an APP should bring up a dialogue box asking for a command line, putting /h or nothing should bring up the command list - for example ZIP uses this to pass the files info.
PRG denotes a GEM app and so you can not pass any info, there is in later GEM versions a GPP extension which allows info to be passed to GEM apps.
ACC progs need to be in the root and are accessories which gives limited multitasking. Some ACCs can be changed to PRG so they run as GEM apps so they do not take up memory when not in use.
As for your problem, does an error actually appear? if not does the box asking for info appear?
You could have the extension pointing to another prog which will confuse things, try booting from a disk with no DESKTOP.INF file (or temparaly rename it if on a HD).
What is the APP you are trying to run?
